To all - thanks for the excellent feedback, and to the comments from FrankJ - I believe we are closer than you believe in our thinking. I was not dismissing any of the eventual goals, simply pointing out that they are not necessarily the first things we should do on our road to perfection. All the things you said are essentially true, and yet if we expect them all to happen magically because they are good things - we will be sorely disappointed in 2011. Rather, I'd suggest we accept the reality that most EMRs are still poorly designed from usability, interoperability and reporting perspectives - and do the best we can with the hand we are dealt now, while laying the groundwork for our future ideal world that would have easy to use EMRs which communicate effectively with each other, provide immediate summaries and beautiful reporting capabilities.

Dr. Lyle's recommendations are right on target. I hope that he gets an opportunity to provide his insight to those making policy decisions. Also, all EMR vendors should get someone with Dr. Lyle's acumen to review their products and roadmaps.

#1. The benefits of e-prescribing go way beyond drug-drug interaction checking and a legible prescription. Yes, these are key patient safety measures but mistakes don't end with the physician office key errors can occur in the pharmacy when reading the prescription. Also add in the benefits of sending prescription renewals electronically to a practice, creates efficiencies both with the practice and the pharmacy. Lastly any viable EMR system has the ability to send electronic files.

#2. I agree the first step is to ensure that physicians are "getting them working in each office first," but interoperability is talked about it is focused on exactly what you mention in the article, "we need the relevant information, and often in a summarized format." The creation of interoperability specification around Continuity of Care Documents (CCD) is exactly what you're looking for, pulling patient problems, medications and recent lab results when needed. Finally your national prescription registry is being developed by using the exact e-prescribing functions that you so eloquently dismiss in your first area of discussion.

#3. Why accept current reporting standards when we know that using an EMR can do it more efficiently. The standards aren't there yet, but they will be based off of the meaningful use requirements and certification criteria that are being created.

Practical, logical, creative...Dr. Lyle, what are you trying to do here: disrupt the whole ARRA/HITECH/Meaningless Use discussion here with such disruptive, I'd dare say virtually anarchistic, commentary?
